Reliance Care Solutions ("Reliance Care," "we," "us") is a licensed Ohio home care and DODD services provider headquartered in Toledo, OH. This policy explains how we collect, use, and protect information you share through our website, intake forms, and care tools.
Information we collect
- Contact details you provide: name, phone, email, ZIP code.
- Care context you choose to share through our tools (eligibility quiz, cost estimator, care plan builder, ZIP coverage checker): waiver status, county, hours needed, general care needs.
- Technical data automatically collected when you browse: device type, browser, pages visited, and similar analytics.
- Cookies for site functionality and (with your consent) analytics. See the Cookies section below.
We do not ask for Social Security numbers, Medicaid IDs, or medical records through our website. Anything you choose to share is treated as a clinical note: stored securely and shared only with the coordinator working your case.
How we use information
- To respond to your inquiry and coordinate care or a free assessment.
- To generate the PDF reports our care tools produce on your device.
- To improve our services, content, and coverage decisions.
- To comply with legal, regulatory, and Ohio Medicaid waiver obligations.
We never sell your information. We do not share it with marketing partners.
HIPAA-aware handling
Reliance Care is a HIPAA Covered Entity for the clinical services we deliver. Information you submit through this website becomes Protected Health Information (PHI) once it is associated with your care. We apply administrative, physical, and technical safeguards consistent with HIPAA's Privacy and Security Rules, including encryption in transit, role-based access, and audit logging.
How we share information
We share information only with:
- Our employees and contracted caregivers, on a need-to-know basis.
- Vendors who help us operate the website and store data securely, under written confidentiality and HIPAA Business Associate Agreements where applicable.
- Government agencies (Ohio Department of Medicaid, ODA, DODD, county boards) when required to bill, document, or comply with a waiver.
- Law enforcement or courts when legally compelled.
Cookies & analytics
We use a minimal set of cookies to keep the site working (essential) and, if you accept, to understand how visitors use the site so we can improve it (analytics). You can change your choice anytime by clearing the reliance-cookie-consent entry in your browser storage; the banner will reappear.
SMS and text messaging

These messages include care coordination, appointment reminders, and intake follow-up. Message frequency varies. Msg & data rates may apply. Reply STOP to opt out at any time, or HELP for help. Consent is not a condition of any purchase or service.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors that support our services (for example, customer support and messaging delivery providers such as GoHighLevel and Twilio) is permitted, and those parties are bound by written agreements to keep your data confidential and to use it only to deliver the messages you have asked for.

Your choices
