How to Become a Paid Family Caregiver in Ohio (2026 Guide)

The short answer

Ohio does pay family members to provide care, primarily through the SELF waiver's participant-direction option, the DODD waiver's consumer-direction, and DODD's self-directed services. Adult children, grandchildren, and (in some cases) spouses can be paid. Rates run roughly $14-$22/hour depending on the waiver and county.

The four paths

1. SELF waiver participant-direction (DODD). The person receiving care directs their own budget and can hire family members, including adult children. Spouses and legal guardians of minors are typically not paid providers.

2. DODD waiver consumer-direction. Under 60 with significant medical need. The participant chooses providers, including some family members, through a Financial Management Service.

3. IO waiver with self-directed budget. More flexibility, but requires county BDD approval of the IO slot first.

4. Structured Family Caregiving (Veterans). For veterans receiving private pay, family caregivers in a shared home can be paid a daily stipend.

Who can be paid in Ohio

Adult children, siblings, grandchildren, nieces, nephews, and friends: generally yes, on most participant-directed plans.

Spouses: limited, depends on the waiver and the participant's age. Always allowed for veterans' programs.

Legal guardians of minor children: not paid as direct-care providers under most Ohio waivers.

The single biggest myth: families think they have to quit their job to be the caregiver. Most of our paid family caregivers work part-time hours that fit around another job.

How to actually start (Lucas County)

Step 1: Call your county Board of DD (Lucas County BDD: 419-380-4033) if the person you care for has a developmental disability, or the Area Office on Aging of NW Ohio (419-382-0624) if they are 60 or older.

Step 2: Request a free Level of Care assessment. This is the gatekeeper for waiver eligibility.

Step 3: Tell the assessor you want to participant-direct (or consumer-direct) the budget and that you have a family caregiver in mind.

Step 4: A Financial Management Service (FMS) issues paychecks, files payroll taxes, and handles the W-2 at year end. You do not have to set up an LLC.

Three routes to get paid in Ohio

Route 1: Agency-employed. You become a State Tested Nurse Aide (STNA) or direct care worker and an agency like Reliance employs you. We handle taxes, background checks, training, and EVV. Steadiest pay, full W-2.

Route 2: Consumer Direction (participant-directed). The waiver participant becomes the employer of record through a Financial Management Service. Higher hourly rate but you handle scheduling and EVV yourself. Available under PASSPORT, DODD waiver, and DODD waivers.

Route 3: Structured Family Caregiving. A specific program where the primary caregiver lives with the participant and receives a daily stipend. See our SFC post for who qualifies.

Spousal pay rules side-by-side

PASSPORT: spouses cannot be paid caregivers. ODA rule. Adult children, siblings, and other relatives can be paid.

DODD waiver: spouses generally cannot be paid except in extraordinary circumstances approved case-by-case.

IO Waiver (DODD): spouses CAN be paid in limited cases. See Ohio Administrative Code 5123-9-04 and ask your SSA.

Level One and SELF: similar to IO but with different cap structures.

How much does Ohio pay family caregivers?

Rates run roughly $14-$22 per hour depending on the waiver, the county, and the level of care. Veterans' Structured Family Caregiving pays a daily stipend ($1,800-$3,400/month typical) plus a tax-free caregiver allowance.

Are paid family caregivers W-2 employees or 1099 contractors?

Under most Ohio waivers, paid family caregivers are W-2 employees of a Financial Management Service (FMS), not 1099 contractors. The FMS handles payroll, taxes, and unemployment insurance.

Can I be paid to care for my spouse in Ohio?

Limited. Spouses are generally not paid providers under PASSPORT or IO. They can be paid under some DODD waiver consumer-direction arrangements and under the Veterans' private pay Structured Family Caregiving program.

Will being a paid caregiver affect my own benefits?

It is taxable income. It can affect SSI, food assistance, and subsidized housing. Run the numbers with a benefits counselor before you sign up. The Area Office on Aging offers free benefits counseling.

Do I need a license to be paid as a family caregiver?

Not always. For personal care under PASSPORT or DODD waiver you generally need to be hired through a certified agency or registered with an FMS. Some services (skilled nursing, behavioral support) require licensure.

What about taxes?

If you are agency-employed, the agency handles W-2 and withholding. If you are participant-directed, the FMS issues you a W-2 and withholds Social Security and Medicare. Either way it is taxable income.

Can I do this and keep my SNAP/SSI?

It depends on the amount earned. Caregiver income counts toward eligibility. Talk to your county JFS office before you start. We have seen families lose food assistance because nobody checked.
