How Long Does Ohio Medicaid Waiver Approval Take? Real Timelines for 2026

Most Ohio DODD waivers approve in 30 to 90 days. PASSPORT averages 30-60 days in Lucas County. MyCare Ohio and the DODD waiver run 45-90. DODD waivers (IO, SELF, Level One) depend on county Board of DD waitlists and can take 60-180 days or longer for IO.

Timelines by waiver

PASSPORT (adults 60+): 30-60 days from application to first authorized service in most NW Ohio counties.

DODD waiver (adults under 60 with significant medical need): 45-90 days, longer if level-of-care documentation is incomplete.

MyCare Ohio (dual-eligible Medicaid + Medicare in participating counties including Lucas): 30-60 days to enroll plus the plan's internal authorization, usually another 2-3 weeks for the first authorized service.

IO (Individual Options) waiver: depends on funding availability at your county Board of DD. Some counties have months-long waitlists; emergency situations are prioritized.

SELF and Level One waivers: typically 30-90 days when funding is available at the county BDD.

The four things that slow approvals down

Missing or outdated medical documentation for the Level of Care assessment. A current physician note describing daily care needs is the single most common cause of delay.

Asset verification at the county Job and Family Services office. Closing accounts, transferring property, or scrambling to qualify mid-application all add weeks.

Backlog at the assessing agency, particularly in late spring and early fall.

Choosing a provider only after approval. You can name your provider on the application, which lets the agency authorize services on day one instead of week three.

A waiver-experienced provider files the referral, chases the assessment date, gathers the medical paperwork, prepares the asset documentation checklist, and bridges with private-pay coverage so you do not have a gap in care. The right partner usually shaves 2-4 weeks off the timeline.

The four-step timeline (most families)

Step 1: Medicaid application. File at Ohio Benefits or through your county Job and Family Services office. Lucas County JFS averages 30 days for a determination.

Step 2: Level of Care assessment. For PASSPORT, Area Office on Aging of Northwestern Ohio schedules the in-home visit. For DODD waivers, Lucas County Board of DD handles assessment.

Step 3: Care plan approval. Case manager builds the plan and submits to the Ohio Department of Medicaid for authorization. 5-10 business days when complete.

Step 4: Provider start. Once authorization is in our system, we can typically have a caregiver in the home within 48-72 hours.

Can care start before Medicaid is approved?

Yes, on a private-pay basis. Many families bridge with private pay during approval and convert to Medicaid billing once authorization is in place. A good agency will not change caregivers when that transition happens.

Why is the IO waiver waitlist so long in some Ohio counties?

IO is the most expensive DODD waiver. County Boards of DD allocate IO slots based on annual funding and prioritize emergency need. Ask your county BDD service coordinator about the SELF or Level One waivers as interim options.

Can I appeal a denial?

Yes. Every Ohio DODD waiver decision is appealable through the state hearing process. You typically have 90 days to request a hearing from the date of the denial notice.

Can I get services during hospital discharge faster?

What if I am already on Medicare?

Medicare does not cover long-term home care. You still need DODD waiver enrollment for ongoing personal care. MyCare Ohio is the dual-eligible option that combines both.
